Alnylam announces positive results from Phase 12 study of lumasiran for treating

(Editor)Oct 4 2018Alnylam Pharmaceuticals, Inc., the leading RNAi therapeutics company, announced today updated positive results from its Phase 1/2 clinical study of lumasiran, an investigational, subcutaneously administered RNAi therapeutic targeting glycolate oxidase (GO) for the treatment of primary hyperoxaluria type 1 (PH1). Results were presented at the 2018 European Society for Paediatric Nephrology (ESPN) Annual Meeting on October 4 in Antalya, Turkey. The Company also announced the initiation of ILLUMINATE-A, a global Phase 3 pivotal trial of lumasiran in children and adults with PH1. The study will enroll approximately 30 patients and is designed in alignment with FDA with a primary endpoint based on reduction of urinary oxalate at six months. Alnylam expects to report topline results from ILLUMINATE-A in late 2019 and, if positive, submit filings for regulatory approval starting in early 2020.New results from the Phase 1/2 study were as of a data cut-off date of August 15, 2018. Lumasiran demonstrated a mean maximal reduction in urinary oxalate of 75 percent (range: 43-87 percent) relative to baseline across cohorts dosed at 1 mg/kg monthly or 3 mg/kg monthly or quarterly (N=20). The mean reduction relative to baseline was 66 percent when measured 28 days post last dose. All patients (100 percent) achieved oxalate lowering to less than 1.5 times the upper limit of normal (less than 0.69 mmol/24hr/1.73m2). Among patients receiving 3 mg/kg monthly or quarterly doses of lumasiran (N=12), 83 percent achieved urinary oxalate levels within the normal range (less than 0.46 mmol/24hr/1.73m2). Furthermore, lumasiran-treated patients in all cohorts experienced a mean maximal decrease of 76 percent in the ratio of urinary oxalate to creatinine – a corroborative measure of oxalate reduction that addresses the variability that is inherent to 24 hour urine collections.”We are pleased to present these data that we believe provide a strong foundation for lumasiran as an investigational RNAi therapeutic for the treatment of PH1, a devastating and life-threatening disease caused by overproduction of oxalate that deposits in the kidneys and other tissues. Once the kidneys fail, the only viable therapeutic option is a dual liver/kidney transplant,” said Prof. Pierre Cochat, M.D., Ph.D., Reference Center for Rare Kidney Diseases, Lyon University Hospital, France; President, International Pediatric Nephrology Association (IPNA) and an investigator in the lumasiran study. “Given the profound unmet need in this disease setting, the Phase 1/2 results presented for lumasiran are encouraging, particularly in light of the clinically meaningful effect of lumasiran on lowering urinary oxalate for every patient relative to their baseline and with all patients achieving near normal levels of oxalate.”The Phase 1/2 safety results in patients with PH1 were based on a median study duration of seven months (range: 5 to 14 months) since first dose. As of the data cut-off date, there were no discontinuations from study treatment. Serious adverse events (SAEs) were reported for one patient (33 percent) receiving placebo and five patients (25 percent) receiving lumasiran; none were related to study drug. The placebo patient experienced acute pyelonephritis and kidney stones. The lumasiran patients with SAEs included one patient with vomiting, one patient with abdominal pain, fever and vomiting, one patient with gastroenteritis, and two patients with kidney stones. Adverse events (AEs) were reported in three (100 percent) patients during placebo dosing and 19 (95 percent) patients after lumasiran dosing. The majority of AEs were mild or moderate in severity and were assessed as unrelated to study drug. Injection site reactions (ISRs) were reported in three (15 percent) patients receiving lumasiran. ISRs were mild or moderate in severity and were self-limiting. Lumasiran was not associated with any clinically significant adverse laboratory findings. In patients receiving lumasiran, plasma glycolate levels increased consistent with the pharmacology of lumasiran and results from healthy volunteers in Part A of the Phase 1/2 study. (Editor)Mar 26 2019More than a decade of research on the mda-7/IL-24 gene has shown that it helps to suppress a majority of cancer types, and now scientists are focusing on how the gene drives this process by influencing microRNAs. Published this week in the journal Proceedings of the National Academy of Sciences, the findings could potentially have implications beyond cancer for a variety of cardiovascular and neurodegenerative diseases caused by the same microRNA-driven processes.The study was led by Paul B. Fisher, M.Ph., Ph.D., F.N.A.I., Thelma Newmeyer Corman Endowed Chair in Cancer Research and member of the Cancer Molecular Genetics research program at VCU Massey Cancer Center, chairman of the Department of Human and Molecular Genetics at VCU School of Medicine and director of the VCU Institute of Molecular Medicine (VIMM). The mda-7/IL-24 gene was originally discovered by Fisher, and he and his colleagues have since published a number of studies detailing how the gene can suppress cancer by directly influencing two important mediators of cell death known as apoptosis and toxic autophagy. They have also been developing mda-7/IL-24 viral gene therapies, purified protein treatments and T-cell-delivered therapies that take advantage of these processes to selectively kill cancer cells.”MicroRNAs play decisive roles in a variety of diseases, including cancer. This study shows for the first time how mda-7/IL-24 influences an enzyme critical to microRNA processing, and it provides exciting clues as to how this process could be targeted therapeutically,” says Fisher.The researchers showed that mda-7/IL-24 reduces the expression of an enzyme called DICER, and this effect occurs only in cancer cells. DICER works to process microRNAs for specific cellular functions. The scientists showed for the first time that mda-7/IL-24 down-regulates MITF, which, in turn, down-regulates DICER, the target of MITF.Previous experiments showed a potent bystander effect where mda-7/IL-24 not only killed cancer cells at the primary tumor site but also in distant secondary tumors not directly targeted by the therapy. The bystander effect is mediated, at least in part, by the potent immune activating and anti-growth properties of mda-7/IL-24. (Editor)Jun 24 2019Anyone who has tried to “shop” for hospital services knows one thing: It’s hard to get prices.President Donald Trump on Monday signed an executive order he said would make it easier.The order directs agencies to draw up rules requiring hospitals and insurers to make public more information on the negotiated prices they hammer out in contract negotiations. Also, hospitals and insurers would have to give estimates to patients on out-of-pocket costs before they go in for nonemergency medical care.The move, which officials said will help address skyrocketing health care costs, comes amid other efforts by the administration to elicit more price transparency for medical care and initiatives by Congress to limit so-called surprise bills. These are the often-expensive bills consumers get when they unwittingly receive care that is not covered by their insurers.”This will put American patients in control and address fundamental drivers of health care costs in a way no president has done before,” said Health and Human Services Secretary Alex Azar during a press briefing on Monday.The proposal is likely to run into opposition from some hospitals and insurers who say disclosing negotiated rates could instead drive up costs.Just how useful the effort will prove for consumers is unclear.Much depends on how the administration writes the rules governing what information must be provided, such as whether it will include hospital-specific prices, regional averages or other measures. While the administration calls for a “consumer-friendly” format, it’s not clear how such a massive amount of data — potentially negotiated price information from thousands of hospitals and insurers for tens of thousands of services — will be presented to consumers.”It’s well intended, but may grossly overestimate the ability of the average patient to decipher this information overload,” said Dan Ward, a vice president at Waystar, a health care payments service.So, does this new development advance efforts to better arm consumers with pricing information? Some key point to consider:Q: What does the order do?It may expand on price information consumers receive.The order directs agencies to develop rules to require hospitals and insurers to provide information “based on negotiated rates” to the public.Currently, such rates are hard to get, even for patients, until after medical care is provided. That’s when insured patients get an “explanation of benefits (EOBs),” which shows how much the hospital charged, how much of a discount their insurer received and the amount a patient may owe.In addition to consumers being unable to get price information upfront in many cases, hospital list prices and negotiated discount rates vary widely by hospital and insurer, even in a region. Uninsured patients often are charged the full amounts.”People are sick and tired of hospitals playing these games with prices,” said George Nation, a business professor at Lehigh University who studies hospital contract law. “That’s what’s driving all of this.”Some insurers and hospitals do provide online tools or apps that can help individual patients estimate out-of-pocket costs for a service or procedure ahead of time, but research shows few patients use such tools. Also, many medical services are needed without much notice — think of a heart attack or a broken leg — so shopping simply isn’t possible.Administration officials say they want patients to have access to more information, including “advance EOBs” outlining anticipated costs before patients get nonemergency medical care. In theory, that would allow consumers to shop around for lower cost care.Q: Isn’t this information already available?Not exactly. In January, new rules took effect under the Affordable Care Act that require hospitals to post online their “list prices,” which hospitals set themselves and have little relation to actual costs or what insurers actually pay.What resulted are often confusing spreadsheets that contain thousands of a la carte charges — ranging from the price of medicines and sutures to room costs, among other things — that patients have to piece together if they can to estimate their total bill. It also is unclear how the administration would enforce the rules.Another limitation: The order applies only to hospitals and the medical staff they employ. Many hospitals, however, are staffed by doctors who are not directly employed, or laboratories that are also separate. That means negotiated prices for services provided by such laboratories or physicians would not have to be disclosed.Q: How could consumers use this information?In theory, consumers could get information allowing them to compare prices for, say, a hip replacement or knee surgery in advance.But that could prove difficult if the rates were not fairly hospital-specific, or if they were not lumped in with all the care needed for a specific procedure or surgery.”They could take the top 20 common procedures the hospital does, for example, and put negotiated prices on them,” said Nation at Lehigh. “It makes sense to do an average for that particular hospital, so I can see how much it’s going to cost to have my knee replaced at St. Joe’s versus St. Anne’s.”Having advance notice of out-of-pocket costs could also help patients who have high-deductible plans.”Patients are increasingly subject to insurance deductibles and other forms of substantial cost sharing. For a subset of so-called shoppable services, patients would benefit from price estimates in advance that allow them to compare options and plan financially for their care,” said John Rother, president and CEO  at the advocacy group National Coalition on Health Care.Q: Will this push consumers to shop for health care?The short answer is maybe. Right now, it’s difficult, even with some of the tools available, said Lovisa Gustafsson, assistant vice president at the Commonwealth Fund, which has looked at whether patients use existing tools or the list price information hospitals must post online.”The evidence to date shows patients aren’t necessarily the best shoppers, but we haven’t given them the best tools to be shoppers,” she said.Posting negotiated rates might be a step forward, she said, but only if it is easily understandable.It’s possible that insurers, physician offices, consumer groups or online businesses may find ways to help direct patients to the most cost-effective locations for surgeries, tests or other procedures based on the information.”Institutions like Consumer Reports or Consumer Checkbook could do some kind of high-level comparison between facilities or doctors, giving some general information that might be useful for consumers,” said Tim Jost, a professor emeritus at the Washington and Lee University School of Law.But some hospitals and insurers maintain that disclosing specific rates could backfire.Hospitals charging lower rates, for example, might raise them if they see competitors are getting higher reimbursement from insurers, they say. Alcohol reduction associated with improved viral suppression in women living with HIV

Reduction in alcohol use is a possible strategy to improve health outcomes in women with HIV, with evidence that unhealthy alcohol use (>7 drinks per week or >4 drinks per occasion for women) is associated with poorer adherence to treatment, lower rates of viral suppression, and faster disease progression.Several medications are available on prescription to help reduce drinking, including naltrexone, which is taken as a once-daily pill; however, none have been studied in relation to clinical outcomes in people with HIV. The participants, most of whom were African American and receiving HIV antiretroviral therapy, were followed up for a total of 7 months and encouraged to adhere to their allocated treatment for four months.At the start of the trial all the women met criteria for unhealthy alcohol use, 62% met criteria for an alcohol use disorder, and only 63% had HIV viral suppression. Women in both groups substantially reduced their drinking over the course of the study; only around half of participants reported ongoing unhealthy alcohol use at four months, and this proportion continued to drop even after stopping the medication. Although women taking naltrexone initally showed a greater reduction in drinking than those assigned to placebo, there was no difference between the two groups from four months onwards; at seven months, 64% of women in each group had reduced or quit drinking.Laboratory analysis showed that HIV viral suppression was significantly more common among women who had reduced or quit drinking than in those who continued to drink at unhealthy levels; after 7 months, 74% of those who had reduced or quit drinking had HIV viral suppression, compared with 54% of those who had not cut their alcohol intake.While naltrexone has been shown to be effective in prior studies, the current decrease in drinking regardless of medication assignment suggests that repeated assessments and support that participants receive in a research study could serve to augment treatment success.
